PARENT/GUARDIAN WAIVER AND RELEASE OF LIABILITY

By signing below, I, the undersigned parent or legal guardian of the above-named Child, acknowledge and agree to the following as a condition of the above-named Child participating in a group visit to Hologram Zoo Lehigh Valley, operated by 28Fortune, LLC (“Company”):

Voluntary Participation: I consent to my Child’s participation in a group visit to Company. I understand the activities will include walking through exhibits and interacting with three-dimensional (3D) holographic technology. Participation is voluntary, and I confirm that I have the authority to allow my child to take part.

Assumption of Risk: I understand that attending Company’s exhibits and exposure to 3D holographic technology involves inherent risks. These may include minor physical strain, tripping or falls, sensory effects (such as moving images, lights, or sounds), and possible dizziness or disorientation from using 3D glasses. I agree that my Child and I are assuming all risks of injury, illness, or discomfort that may result from these activities.

Medical Authorization: In the event my Child requires medical attention during the visit, I authorize Company staff to arrange for emergency medical care (including transportation via ambulance if deemed necessary). I understand that any medical expenses incurred for my Child’s treatment are my responsibility. I agree that I will reimburse Company for any costs it incurs in obtaining medical care for my Child.

Behavior and Safety: I will instruct my Child to follow all safety rules and to listen to the instructions of Company staff. My Child is expected to behave respectfully and not endanger themselves or others. Company reserves the right to remove any participant (child or accompanying adult) from the premises for disruptive, inappropriate, or unsafe behavior, without refund. If asked to leave, my Child will promptly comply.

Release of Liability: I, on behalf of myself and my Child, hereby release and hold harmless Company, its owners, employees, volunteers, and affiliates from any and all liability, claims, or demands arising from my Child’s participation in the Company visit. This includes any personal injury, illness, death, or property damage suffered by my Child or myself in connection with the visit. This release applies even if the injury or damage is alleged to be caused by the negligence of Company or its staff, except in cases of gross negligence or willful misconduct.

Indemnification: I agree to hold harmless and indemnify Company from any claim, demand, lawsuit, or expense (including reasonable attorneys’ fees) arising out of my Child’s participation. If my Child, I, or anyone on our behalf makes a claim against Company despite this waiver, I will be responsible for all loss, damages, and expenses incurred by Company, including legal fees.

No Guarantees; Personal Expense: I understand that Company has not made any guarantees about the results or experiences of the visit. I accept that Company is not responsible for any personal property that is lost, stolen, or damaged during the visit. I acknowledge that any expenses I incur in relation to my Child’s participation (for example, travel or medical costs) are my own responsibility, and Company will not reimburse these costs.

Force Majeure: I agree that Company is not responsible for events beyond its control that may affect the visit. This includes events like severe weather, power outages, or other emergencies. I understand that if such a force majeure event occurs, the group visit may be rescheduled or canceled for safety reasons at Company’s discretion, and Company will not be liable for any resulting costs or losses.

Acknowledgment of Understanding: I have read (or had the opportunity to read) this entire waiver and understand its contents. I am signing it freely and voluntarily. I understand that by signing this document, I give up certain legal rights that I or my Child might otherwise have, including the right to sue Company for injuries or losses. I agree that this release is binding on me, my Child, and any legal representatives.
